It will then go to the box below. From there start to type in the name of your GP practice making sure that the name and address is correct. (Grove Medical Practice for example
It should say that ‘Sleepstation is available on the NHS in your area’.
From there, you can provide your details and self refer yourself. It may ask you some further questions.
